Funeral Facility Use Request
This form should be used when requesting use of the facilities of Lifehouse Community Church in Urbandale, Iowa. This form serves to collect basic request information.

Affirmation of Intended Use
I affirm that:
1. I understand that Lifehouse Community Church does not allow its facilities to be used in a way that contradicts its Statement of Faith or by persons or groups holding beliefs that contradict the Church’s Statement of Faith.
2. To the best of my knowledge the purpose for which I am requesting the use of Lifehouse Community Church facilities will not contradict the Church’s Statement of Faith, and I commit to promptly disclose any potential conflict of which I am aware or become aware to Church staff.
3. I am not aware of any beliefs that are professed by me or the church I regularly attend and represent contradict the beliefs of the Church. I agree to promptly disclose any potential conflicts in belief to Church staff.
4. I understand that Lifehouse Community Church does not allow its facilities to be generally available to the public, and that my use of these facilities is subject to the pastor’s approval, which is conditioned in part on my/our agreement to the requirements in the “Church Facility Use Policy,” a copy of which I have read and understand.
5. I understand that I will be responsible for any damages to the Facilities of Lifehouse Community Church resulting from this proposed use of Facilities.
6. Lifehouse Community Church believes, and I/we agree, that disputes arising under the use of the Church’s Facilities are to be worked out between parties with mediation and arbitration before any litigation.
7. Any fee for using the Lifehouse Community Church’s facility is documented by a fee agreement and is incorporated into this request.
Statement of Understanding & Agreement
I have read and agree to the above terms and conditions regarding my request to use the facilities of Lifehouse Community Church. I also affirm and agree to the above statements under Intended Use. I understand that actual scheduling is subject to availability and that Lifehouse Community Church reserves full rights in extending or refusing any requests for use and proposed dates at its discretion; with or without explanation. I also attest that I will bear the full legal and financial responsibility should I be found attempting to defraud, defame, entrap, or otherwise damage the function, purpose, or reputation of Lifehouse Community Church, its staff and/or leaders in association to this facility use request.
